ELIGIBILITY

You qualify for Home Health Care services if you:

• Are unable to leave your home without assistance, and
• Have a skilled health need resulting from illness or injury that requires the care of a registered nurse or rehabilitation therapist, and
• Have the care ordered by your physician.

Services Provided through the HOME Health include:

Skilled Nursing, Home Health Aides, Physical Therapy, Speech, Pathology, Occupational Therapy, I. V. Therapy. As well as In Home Services, Personal Care, Homemaker Chore & Respite.

Equipment and Supplies:

We provided assistance in obtaining needed equipment such as wheel chairs, beds, canes, and walkers as well as oxygen and oxygen supplies for use in the home and other supplies you may need such as dressings and catheters.

Home Health Care Costs may be Covered by:

Private Health Insurance programs. Various government programs such as Medicare, Medicaid or Veteran’s Insurance. Workers’ Compensation. Automobile Insurance.
